A first-time applicant laboratory may receive a notice stating the following: The accreditation decision for your laboratory has been delayed for the reasons outlined below. Once resolved, a one-year provisional accreditation will be granted. In this case, the laboratory is potentially eligible for a one-year provisional grant due to non-compliance with quality assurance, continuing medical education and/or credentialed Technical Director Standards; however, the laboratory has also failed to comply with certain Standards for additional issues, such as cases and/or reports. The letter will first outline these deficiencies, or delay issues, along with the applicable Standard and the information needed to rectify these issues. One-year provisional issues are listed next, with relevant Standards and the list of required additional documentation. In order for the laboratory to take advantage of the one-year provisional grant, the delay issues must be resolved before the expiration of the [potential] one-year provisional grant period noted in the letter. For this reason, laboratories may wish to submit the requested delay information first, in order to be awarded provisional accreditation. Additional documentation to correct provisional issues may be submitted later. If the laboratory does not resolve the delay issues prior to the one-year date given, they will have forfeited their chance to become provisionally granted, and must submit ALL requested material to be considered for [the remainder of] the full three-year grant. Please refer to Delay Material: Important Facts for complete submission deadline information. The Board of Directors may determine that a site visit is required in order to better assess the laboratory and determine the final accreditation decision. In this case, the site visit notification letter simply informs the laboratory that a visit is requested and provides instructions for arranging the visit. No application for accreditation will ever be initially denied; laboratories would first receive a notification of a delay or a site visit. A notification of denial may be sent if a laboratory fails three times to demonstrate substantial compliance to The ICAEL Standards. Additional Information
