REPRESENTATIVE CASE STUDY REQUIREMENTS


Adult Transesophageal Representative Cases:

One representative Adult TEE case and its final report for each physician that performs TEE.

1 case per physician that includes an indication or finding of a greater than mild mitral regurgitation or source of emboli. Of the total submitted, at least one case must demonstrate a finding of greater than mild mitral regurgitation.

All representative cases should have been performed within the last twelve months.


FORMATS FOR SUBMISSION

Submit as many TEE cases as possible on a single CD, DVD, or standard (not Super) VHS tape. ***All disks MUST include a reader/viewer that allows the disk to be opened and read on any PC (DICOM format cannot necessarily be opened on any PC).***
NOTE TO LABS: Beginning January 1, 2010, VHS tapes will no longer be an accepted format for submitting case studies. These cases will need to be transferred to a digital format (CD or DVD) prior to submission to the ICAEL.
